Disclosure Statement on the Processing of Personal Data Collected via Forms
Our clinic requests certain personal data (such as name, surname, password, email address, and mobile phone number) in order to fulfill its legal obligations arising primarily from the Law on the Protection of Personal Data No. 6698 and related legislation, and to manage relevant processes. Your personal data will be processed based on your explicit consent, solely for the purposes stated in this KVKK Disclosure Text, and will be stored in a secure environment that is not accessible to the public.
Purpose of Processing Personal Data
Within our clinic, personal data may be processed for purposes such as planning and executing commercial activities, providing information to authorized public institutions and organizations as required by law, resolving consumer complaints in matters outside our area of expertise, sending commercial electronic communications if you provide explicit consent, ensuring that activities are carried out in compliance with our internal procedures and applicable legislation, planning and executing audit activities, supporting corporate sustainability processes, protecting the company’s reputation, managing requests and complaints, and planning and executing corporate governance and communication activities.
Your personal data may be processed through automated or non-automated means such as collection, recording, storage, preservation, modification, and reorganization, in compliance with the principles of lawfulness, fairness, proportionality, purpose limitation, and retention only for the period required by law or the purpose of processing. The collected data will not be shared with third parties, used for unrelated purposes, or sold without your knowledge or instruction.
Transfer of Personal Data
Your personal data may be transferred, in compliance with legal principles and limited to the purposes stated above, to the Ministry of Trade, domestic and international public or private institutions and organizations, companies, consultants, business partners, group companies, suppliers, subcontractors, and other authorized entities as permitted by Turkish Commercial Law and relevant legislation, as well as to official authorities upon lawful request.
Personal data may be transferred without explicit consent in accordance with Articles 5 and 8 of the Law on the Protection of Personal Data.
Method of Collecting Personal Data
Your personal data may be collected through our website in written, verbal, or electronic form.
Deletion, Destruction, or Anonymization of Personal Data
In accordance with Article 7 of the Law, even if personal data has been processed in compliance with the law, it will be deleted, destroyed, or anonymized by our clinic ex officio or upon the request of the data subject when the reasons requiring its processing no longer exist.
If you request the deletion or destruction of your personal data, your request will be evaluated and concluded within 30 days:
- a) If all conditions for processing personal data have ceased, your data will be deleted, destroyed, or anonymized.
b) If your data has been shared with third parties, they will also be informed and necessary actions will be ensured.
c) If legal conditions for processing still exist, your request may be rejected with justification.
Your Rights Under the Law
Under Article 11 of the Law, as a data subject, you have the right to:
- Learn whether your personal data is processed,
- Request information if your data has been processed,
- Learn the purpose of processing and whether it is used accordingly,
- Know third parties to whom your data is transferred domestically or abroad,
- Request correction of incomplete or incorrect data,
- Request deletion, anonymization, or destruction of your data,
- Request notification of such actions to third parties,
- Object to results arising against you through automated processing,
- Claim compensation for damages caused by unlawful processing of your data.
Non-personal data refers to information that does not identify you personally and may be used for any purpose and shared with third parties without consent.
